REFERRALS
We want to continue providing excellent health care in patient’s home. Innova Home Health provides state of the art health care to patients in their home. The process to begin care with us is fast and easy. Simply fill out one of our referral forms and fax it to our intake department. Please remember to include a H&P and notes from the patient’s last visit. If you have any questions, feel free to call and speak with our intake department. We are happy to assist in any way to make the process efficient for you.
